Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join ChristianaCare's HomeHealth team. This is a fantastic opportunity to provide skilled nursing care to geriatric patients and contribute to goal-oriented care plans in New Castle County, DE. The successful candidates will work flexible hours, including day or night shifts, weekends, and holidays.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans aligned with available resources.
- Participate in developing discharge plans.
Requirements
- High School Diploma or GED required.
- Must have a current DE LPN license or compact state license.
