We are looking for a dedicated LPN to join Genesis Health, a leading healthcare provider that offers a wide range of healthcare services. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure proper patient care and reimbursement.

Position Highlights

• Lead the nursing center's Resident Assessment Instrument (RAI) process in compliance with guidelines, including MDS completion and electronic transmission.
• Create individualized resident Care Plans, addressing resident needs and expected outcomes as per regulations.
• Provide training on clinical content and electronic MDS completion to new interdisciplinary team members.

Requirements

• Active LPN license.
• Prior experience in healthcare facilities.
• Familiar with nursing practices, medical procedures, and long-term care regulations.
• Proficient in computer applications, preferably AccuMed.
• Knowledge of RAI/PPS processes and previous MDS Coordinator experience are advantageous.
• Current BLS/CPR certification.
