Job Description

Genesis HealthCare is looking for a dedicated Minimal Data Set Coordinator - LPN to join our team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure proper patient care and nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process following guidelines for MDS completion, encoding, and electronic State transmission.
• Contribute to the development of individualized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic completion of applicable MDS items to new team members.

Requirements

• Active LPN license.
• Prior experience in hospital or long-term care settings.
• Familiarity with nursing practices, medical procedures, and relevant regulations.
• Proficiency in Windows applications; AccuMed experience is a plus.
• Previous MDS Coordinator experience preferred.
• Current BLS/CPR certification.
