Job Description: Nurse-LPN MDS Coordinator

We are looking for a dedicated Nurse-LPN MDS Coordinator to join Genesis Healthcare, a leading provider of healthcare services. The LPN MDS Coordinator plays a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs). Responsibilities include direct involvement in the RAI process, developing resident Care Plans, and training team members on MDS completion. The ideal candidate must hold an active LPN license, possess relevant experience in healthcare, and demonstrate proficiency in RAI/PPS processes.

Position Highlights:

• Lead the RAI process and ensure accurate completion and transmission of MDS.
• Create individualized resident Care Plans and identify resident needs and outcomes.
• Train interdisciplinary team members on clinical content and electronic MDS completion.

Requirements:

• Active LPN license with experience in long-term care or healthcare facilities.
• Familiarity with nursing practices, medical procedures, and long-term care regulations.
• Proficiency in computer applications, Windows, and AccuMed experience preferred.
• Prior exposure to MDS completion and Care Plans.
• Current BLS/CPR certification.
