Nurse Practitioner (NP) - Senior Living

Job Description

OnSite Healthcare and Wellness, LLC is seeking a compassionate and motivated Nurse Practitioner (NP) to join our team and provide high-level medical and mental health care to patients in Assisted Living, Independent Living, Skilled Nursing Facilities, and home settings. Our ideal candidate will have excellent communication and documentation abilities, strong patient assessment skills, and a patient-centric work ethic.

Responsibilities

• Manage acute, subacute, and primary care needs of the elderly population.
• Collaborate with the OnSite team, nursing home staff, residents, and families.
• Follow up with patients to assess the effectiveness of interventions.
• Provide education on best practices and integrate non-pharmacological treatments.

Requirements

• Master of Science in Nursing with certification (FNP, ANP, AGNP, AGACNP, AGPCNP, or ACNP).
• Active South Carolina License and DEA registration.
• 1+ year of experience in a similar role.
