Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) program.
• Hold a valid Physical Therapy (PT) license for New York.
• Conduct home visits to assess patients' medical conditions and provide direct care.
• Proficient in EMR documentation.
• CPR Certification.
