• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license in New York.
• Conduct home visit assessments of referred patients to provide direct care and assess medical conditions.
• Proficient in EMR documentation.
• CPR Certification.
