Responsibilities

• Conduct home visit Physical Therapy assessments to evaluate medical conditions and functional capabilities.
• Provide direct patient care to support rehabilitation and improve patients' quality of life.
• Proficiently document patient information using EMR systems.

Requirements

• Graduate of an accredited Physical Therapy (PT) program.
• Current, unencumbered Physical Therapy (PT) license for New York.
• CPR Certification.
• Experience in EMR documentation preferred.
