Requirements

• Graduated from an accredited Physical Therapy (PT) college or university.
• Unencumbered Physical Therapy (PT) license for New York.
• Proficiency in conducting home visit Physical Therapy assessments and providing direct patient care.
• Experience in EMR documentation preferred.
• CPR Certification required.
