Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered PT license for New York.
• Conduct home visit assessments, assessing medical conditions and rehabilitation potential.
• EMR documentation proficiency preferred.
• CPR Certification required.
