Primary Care Physician (Bilingual Spanish)

Job Description

Esperanza Health Centers is seeking a dedicated Primary Care Physician (Bilingual Spanish) to provide essential medical care services to older adult participants of our Program of All Inclusive Care for the Elderly (PACE). As a primary care physician, you will play a crucial role in promoting healthy lifestyles and ensuring the well-being of our participants.

Primary Duties and Responsibilities

1. Manage primary care medical practice in PACE Center, participants' homes, and post-acute facilities.
2. Coordinate with the Interdisciplinary Team (IDT) to develop individualized plans of care for each participant.
3. Participate in quality improvement activities and ensure sound utilization management of participant care.
4. Maintain communication with partner facilities to ensure continuity of care.

Requirements

• Bilingual (English and Spanish) proficiency.
• Board certification in primary care; specialization in geriatrics preferred.
• Valid physician license in the State of Illinois.
• Minimum 1 year of experience working with older adults.
• Valid driver's license and automobile insurance.
