Duties and Responsibilities:
- Deliver expert care to medically fragile patients.
- Coordinate care with families and healthcare providers.

- Administer medications and treatments as prescribed.
Requirements:
- DE LPN or RN license with healthcare experience.
- Attending orientation and patient field training.
- Previous home health or Long Term Care experience beneficial.
- Prefer tracheostomy and ventilator expertise.
