ChristianaCare HomeHealth is seeking a Full Time Registered Nurse (RN) Field Nurse to provide skilled nursing care and treatment to geriatric patients in the community. The successful candidate will work in the Newark, DE area, with day shift hours, one weekend a month, 2 holidays a year, and two on-call shifts a month. Responsibilities

• Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
• Develop and implement patient-centered care plans consistent with available resources.
• Ensure IV proficiency and perform IV functions.
• Develop discharge plans for patients.

Requirements

• BSN required; ADN candidates must obtain BSN within three years of employment.
• Current DE RN license or compact state license.
• 2 years of Home Health RN experience within the last 12 months.
• Previous Home Healthcare and Med-Surgical experience preferred.
