Job Description
We are looking for a dedicated Registered Nurse (RN) Field Nurse for Home Care to join our team at ChristianaCare. As a Full-Time RN, you will be responsible for providing skilled nursing care to geriatric patients in the community, developing care plans, and ensuring patient-centered treatment. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients at their homes.
- Develop and implement care plans that are goal-oriented and patient-centered.
- Create discharge plans for patients in coordination with the healthcare team.
Requirements
- BSN required; ADN candidates must obtain a BSN within three years of hire.
- Current DE RN license or compact state license.
- Minimum 2 years of recent HomeHealth RN experience.
- Proficiency in IV procedures or willingness to train.
