Job Description
Atrium Health is looking for a dedicated and compassionate Registered Nurse (RN) to join the Home Health team in Charlotte, North Carolina. As a member of our team, you will play a crucial role in providing world-class healthcare to adult and geriatric patients in the comfort of their homes.
Job Summary
The Registered Nurse will serve as a case manager, overseeing post-acute services in the home setting. Responsibilities include providing direct patient care, supervising LPNs and HHAs, ensuring care coordination, and maintaining quality patient care and documentation.
Essential Functions
- Manage assigned patient caseload following established care plans and physician orders.
- Collect and analyze patient data for effective resolution and care coordination.
- Provide oversight of care plan implementation and multidisciplinary team collaboration.
- Perform treatments and evaluate patient progress towards goals.
Requirements
- Graduate from an accredited School of Nursing, BSN preferred.
- Valid RN license and BLS certification.
- Additional training or certifications as required.
- Valid driver's license.
