Registered Nurse (RN) - Home Health - $32-58 per hour

Job Description

Atrium Health is looking for a dedicated and compassionate Registered Nurse (RN) to join their Home Health team in Charlotte, North Carolina. In this role, you will be responsible for overseeing post-acute services in patients' homes, ensuring high-quality care and patient satisfaction.

Responsibilities

• Manage an assigned patient caseload, providing direct patient care and supervision of LPNs and HHAs.
• Coordinate care with a multi-disciplinary team and other healthcare providers to ensure effective, timely services.
• Develop and implement a plan of care to promote health and safe outcomes for patients.

Requirements

• Graduate from an accredited School of Nursing, BSN preferred.
• Current RN license and BLS certification required.
• Previous experience in home health or similar setting preferred.
