• Conduct home visits to provide direct patient care and assess medical conditions, functional capabilities, limitations, and rehabilitation potential.
• Proficiently document assessments in EMR systems.

Requirements

• Graduation from an accredited Physical Therapy program.
• Active Physical Therapy license for New York.
• CPR Certification.
