Rehabilitation Therapist Responsibilities:

• Perform home visit assessments on referred patients to evaluate medical conditions, functional capabilities, and potential for rehabilitation.
• Provide direct patient care to improve physical conditions and enhance mobility.
• Ensure accurate documentation using EMR systems.
• Maintain CPR Certification.

Rehabilitation Therapist Requirements:

• Graduate from an accredited Physical Therapy college or university.
• Hold an unencumbered Physical Therapy license for New York.
• Experience with EMR documentation preferred.
• Flexible scheduling and availability for home visits.
