Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered PT license for New York.
• Perform home visit assessments, provide direct patient care, and assess medical conditions for rehabilitation potential.
• Proficient in EMR documentation.
• Hold CPR Certification.
