Responsibilities
- Conduct home visit assessments to evaluate medical conditions and rehabilitation potential.
- Provide direct patient care and develop personalized treatment plans.
- Proficiently document patient progress using EMR systems.
- Maintain CPR certification and adhere to licensing requirements.
Requirements
- Graduation from an accredited Physical Therapy program.
- Valid, unencumbered Physical Therapy license in New York.
- Experience with home-based patient care is preferred.
- Proficiency in EMR documentation (Hello Note experience a plus).
