Job Description

We are looking for a compassionate and dedicated RN Care Coordinator, Ambulatory Care to join our team at Denver Health. In this role, you will be responsible for coordinating care for a group of patients, ensuring quality outcomes through collaboration and assessment. You will work with multidisciplinary teams, patients, and families to facilitate patient care, promote health literacy, and support timely treatment.

Responsibilities

• Create patient-centered goals and care plans.
• Coordinate care delivery within clinic and healthcare settings.
• Advocate for patient benefits and overcome barriers to care.
• Support patients and caregivers in health education and skill development.

Requirements

• Associate's Degree in Nursing and RN license.
• 3 years of clinical experience in healthcare settings.
• Proficiency in case management principles and care coordination.
• Strong communication, problem-solving, and cultural competency skills.
