Job Summary

We are seeking a dedicated RN Care Coordinator to join Denver Health Medical Plan. This role involves coordinating care for a group of patients through collaboration with healthcare teams to ensure quality outcomes within care coordination parameters. The Care Coordinator will be responsible for facilitating appropriate care, assessments, patient education, and interdisciplinary team support.

Essential Functions

• Implement evidence-based care plans.
• Collaborate with healthcare teams and patients to establish measurable goals and outcomes.
• Provide nursing care, education, and advocacy.
• Supervise and support clinic team members.
• Maintain accurate patient records and reports.

Requirements

• Associate's Degree in Nursing.
• 3 years clinical experience and 1-3 years in care coordination.
• Active RN license and BLS certification.
• Knowledge of case management principles and effective problem-solving skills.
• Proficiency in windows-based computer programs.
