Job Description
We have an exciting opportunity for a PRN Social Worker at LifeBridge Health's Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, and more.
Responsibilities
- Conduct psychosocial assessments and provide counseling to address patient and family needs.
- Assist with discharge planning and support patients through difficult life transitions.
- Collaborate with healthcare teams to ensure comprehensive and holistic care.
Requirements
- Bachelor's degree and 1-3 years of relevant experience.
- LBSW, LMSW, or LGPC certification.
- Strong knowledge of social work theory and practice.
