SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges they may be facing. This role involves assisting with adjustment to institutional living, financial concerns, family relationships, medical understanding, aging issues, discharge planning, and counseling related to end-of-life care.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address common challenges such as adjustment to institutional living, financial concerns, family relationships, and more.
• Offer counseling related to end-of-life care.
• Collaborate with multidisciplinary teams to ensure holistic patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
