SOCIAL WORKER PRN

Job Description

We are looking for a passionate and dedicated Social Worker on a PRN basis to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families. Your responsibilities will include addressing various issues such as adjustment to institutional living, family relationships, discharge planning, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common problems related to adjustment, financial concerns, family relationships, and aging.
• Offer counseling and support for issues including medical diagnoses, treatments, and discharge planning.

Requirements

• Bachelor's degree and 1-3 years of experience in social work.
• LBSW, LMSW, or LGPC certification required.
• Working knowledge of social work theory and practice.
