Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. This position, based in Baltimore, MD at Levindale Hospital, involves providing clinical social work services and psychosocial assessments to patients and their families. The role requires addressing a range of common challenges such as adjustment to institutional living, financial concerns, family dynamics, and end-of-life counseling.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address issues related to adjustment, financial concerns, family relationships, and end-of-life counseling.
• Collaborate with healthcare teams to ensure comprehensive care for patients.
• Participate in discharge planning and coordinate community resources.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
