SOCIAL WORKER PRN

Job Description

We are looking for a compassionate PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide essential clinical social work services to patients/residents and their families, addressing various challenges such as adjustment to institutional living, financial concerns, and counseling related to aging and end-of-life care.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients/residents and families.
• Address common issues related to medical diagnoses, discharge planning, and end-of-life care.
• Collaborate with healthcare teams to ensure comprehensive patient care.

Requirements

• Minimum of Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
