SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services to patients and families.
• Conduct psychosocial assessments and offer counseling.
• Assist with discharge planning and support patients and families in difficult situations.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in a similar role.
• LBSW/LMSW/LGPC certification.
