Job Description

We are looking for a dedicated SOCIAL WORKER PRN to join our team at LifeBridge Health in Baltimore, MD. This position at LEVINDALE HOSPITAL involves providing clinical social work services and psychosocial assessments to patients/residents and families, addressing a variety of common issues related to institutional living, family dynamics, medical procedures, aging, and end-of-life care.

Responsibilities

• Offer clinical social work services to patients/residents and families.
• Address issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life care.
• Provide counseling related to death and dying.
• Assist with discharge planning.

Requirements

• Basic professional knowledge with a Bachelor's degree.
• 1-3 years of experience.
• LBSW/LMSW/LGPC certification.
