SOCIAL WORKER PRN

Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a member of our Allied Health team at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients and families, addressing various challenges related to institutional living, family dynamics, medical procedures, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services to patients and families.
• Address common problems such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
• Assist patients in understanding and accepting medical diagnoses, procedures, and treatments.
• Collaborate with healthcare teams to support discharge planning and promote overall well-being.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of professional experience as a Social Worker.
• Licensed as LBSW, LMSW, or LGPC.
