SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker to join our team at LifeBridge Health on a PRN basis. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various challenges such as adjustment to institutional living, financial concerns, family dynamics, and end-of-life counseling. Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common issues such as adjustment to institutional living, financial concerns, and family relationships.
• Assist with discharge planning and provide counseling related to death and dying.

Requirements

• Bachelor's degree and 1-3 years of experience.
• LBSW, LMSW, or LGPC certification.
• Demonstrated working knowledge of social work theory and practice.
