SOCIAL WORKER PRN

Job Description

We are looking for a dedicated Social Worker (PRN) to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various common challenges they may encounter. This role involves working with individuals to assist with adjustment to institutional living, financial concerns, family relationships, and other issues related to healthcare and well-being.

Responsibilities

• Provide psychosocial assessments to patients and families.
• Address common problems such as adjustment to institutional living, financial concerns, and family relationships.
• Assist patients with understanding and accepting medical procedures and treatments.
• Provide counseling related to aging, discharge planning, and end-of-life issues.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in social work.
• Licensed Social Worker (LBSW, LMSW, LGPC).
