SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families on various common issues. Your responsibilities will include addressing adjustment to institutional living, financial concerns, discharge planning, and counseling on topics related to aging and end-of-life care.

Requirements

• Bachelor's degree or equivalent professional knowledge.
• 1-3 years of experience in social work.
• LBSW, LMSW, or LGPC certification.
