Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a PRN Social Worker, you will provide clinical social work services to patients and their families, assisting with various challenges such as adjustment to institutional living, financial concerns, family relationships, and more.

Responsibilities

• Conduct psychosocial assessments for patients and families.
• Address issues related to medical procedures, aging, discharge planning, and end-of-life counseling.
• Collaborate with healthcare teams to ensure comprehensive patient care.

Requirements

• Bachelor's degree and 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong knowledge in the field of social work theory and practice.
