SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing a range of common problems related to medical conditions, aging, discharge planning, and emotional support.

Responsibilities

• Conduct psychosocial assessments and provide counseling to individuals and families.
• Assist patients/residents with adjustment to institutional living and aging-related changes.
• Offer support in financial concerns, family relationships, and end-of-life care.
• Collaborate with healthcare professionals to ensure comprehensive care for patients.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in a healthcare setting.
• Licensed as a LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
