SOCIAL WORKER PRN

Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying. This role plays a crucial part in supporting individuals during vulnerable times.

Responsibilities

• Provide clinical social work services to patients/residents and families.
• Conduct psychosocial assessments and address common problems affecting individuals.
• Offer counseling related to various emotional and practical challenges.
• Participate in discharge planning processes.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as an LBSW, LMSW, or LGPC.
• Demonstrated knowledge and practice in the social work field.
