Job Description
We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, assisting with various challenges and concerns they may face during their stay. Responsibilities
- Provide clinical social work services to patients and families.
- Assess and address common psychosocial problems related to institutional living, family dynamics, medical conditions, aging, and end-of-life planning.
- Offer counseling and support during difficult transitions and decision-making processes.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- Licensed Social Worker (LBSW/LMSW/LGPC).
- Strong knowledge of social work theory and practice.
