Job Description
We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various concerns and challenges they may face during their healthcare journey. This role involves assisting individuals with adjustment to institutional living, financial issues, family dynamics, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments for patients and families.
- Provide counseling and support for patients dealing with medical conditions.
- Assist with discharge planning and end-of-life counseling.
- Collaborate with healthcare teams to ensure holistic patient care.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of experience in clinical social work.
- Licensed Social Worker (LBSW/LMSW/LGPC).
- Strong interpersonal and communication skills.
