Job Description
We are looking for a dedicated PRN Social Worker to join our compassionate team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical services to patients and their families, addressing various common issues such as adjustment difficulties, financial concerns, family dynamics, and end-of-life planning.
Responsibilities
- Conduct psychosocial assessments and provide counseling.
- Assist patients with discharge planning and coping with medical procedures.
- Collaborate with interdisciplinary teams to support patient well-being.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience as an LBSW, LMSW, or LGPC.
- Strong knowledge of clinical social work theory and practice.
