SOCIAL WORKER PRN

Job Description

We are looking for a passionate PRN Social Worker to join LifeBridge Health in Baltimore, MD, at Levindale Hospital. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, assisting with various issues including adjustment to institutional living, financial concerns, and family relationships.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and end-of-life counseling.
• Collaborate with healthcare teams to support patient well-being.
• Address various social, emotional, and financial concerns of patients.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of social work experience.
• LBSW/LMSW/LGPC certification.
• Strong understanding of social work theory and practice.
