SOCIAL WORKER PRN

Job Description

We are looking for a passionate SOCIAL WORKER PRN to join LifeBridge Health's team at Levindale Hospital in Baltimore, MD. As a SOCIAL WORKER PRN, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various common challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to end-of-life matters.

Responsibilities

• Conduct psychosocial assessments.
• Provide counseling and support to patients and families.
• Assist with discharge planning.
• Address concerns related to aging and end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong interpersonal and communication skills.
