Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges and offering support in times of need.
Responsibilities
- Offer clinical social work services to patients and families dealing with institutional living, financial concerns, family relationships, medical issues, aging, and end-of-life care.
- Conduct psychosocial assessments to identify patient needs and develop appropriate care plans.
- Assist patients with discharge planning and provide counseling on coping with challenging situations.
Requirements
- Bachelor's degree or equivalent professional knowledge in social work.
- 1-3 years of relevant experience as an LBSW, LMSW, or LGPC.
- Strong knowledge of social work theory and practice within a specialized field.
