Social Worker PRN

We are currently seeking a dedicated Social Worker PRN to join our compassionate team at LifeBridge Health in Baltimore, MD. As a Social Worker PRN, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common challenges such as adjustment to institutional living, financial concerns, family relationships, and more.

Responsibilities

• Conduct psychosocial assessments.
• Provide counseling on various issues affecting patients and families.
• Assist with discharge planning and end-of-life counseling.
• Collaborate with healthcare teams to ensure holistic patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong communication and interpersonal skills.
