Job Description

We are looking for a compassionate PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to end-of-life issues.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and families.
• Address adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
• Participate in discharge planning and support patients through difficult life transitions.

Requirements

• Bachelor's degree or equivalent professional knowledge.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
