SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and skilled Social Worker (PRN) to join LifeBridge Health at Levindale Hospital in Baltimore, MD. As part of our Allied Health team, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing a variety of common issues related to institutional living, family relationships, medical procedures, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments for patients and families.
• Provide counseling and support for patients and families facing medical challenges.
• Develop and implement discharge plans.
• Collaborate with healthcare professionals to ensure comprehensive patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of social work experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
