SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. You will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling on end-of-life issues.

Responsibilities

• Offer clinical social work services to patients and families.
• Address issues related to adjusting to institutional living and aging.
• Provide counseling on financial concerns and end-of-life planning.
• Collaborate with the healthcare team on discharge planning.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of relevant experience as an LBSW/LMSW/LGPC.
• Maintain working knowledge of social work practices.
