SOCIAL WORKER PRN

Job Description

We are looking for a passionate and dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker PRN, you will provide clinical social work services and psychosocial assessments to patients and families, addressing common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services to patients and families.
• Conduct psychosocial assessments and address common problems experienced by patients.
• Offer counseling and support related to various emotional and social challenges.
• Participate in discharge planning and collaborate with the healthcare team.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in clinical social work.
• LBSW/LMSW/LGPC certification.
• Strong communication and interpersonal skills.
