SOCIAL WORKER PRN

Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and emotional support. This position involves counseling, discharge planning, and end-of-life support.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services.
• Address the emotional and social needs of patients and families.
• Assist in discharge planning and end-of-life counseling.
• Collaborate with healthcare teams to provide comprehensive care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Ability to provide compassionate and patient-centered care.
