Job Description

We are looking for a dedicated Social Worker (PRN) to join our compassionate team at LifeBridge Health. As a Social Worker, you will provide essential clinical social work services to patients and families, offering support and guidance in various challenging situations.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Address issues such as adjustment to institutional living, financial concerns, and end-of-life care.
• Collaborate with healthcare teams to develop comprehensive treatment plans.
• Assist with discharge planning and offer support during difficult times.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• Strong interpersonal and communication skills.
