Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a wide range of common issues. Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Address challenges such as adjustment to institutional living, financial concerns, and family relationships.
• Assist with discharge planning and offer support related to death and dying.
• Collaborate with healthcare team members to ensure comprehensive patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience as a licensed social worker.
• LBSW/LMSW/LGPC certification.
• Strong communication and interpersonal skills.
